Image & Audio/Video Recording Waiver

The Academy of Music in Grand Rapids occasionally use photos, interviews, and/or video clips of individual or group music instruction in the studio marketing materials, social media, studio owned websites, or for instructional purposes.

This is a request for your permission to include images of you or your child/children, and/or comments from photos or videos now or in the future. The use of the images is strictly controlled for student safety.

Tuition and Fees

 

All payments are processed by authorized debit/credit cards. (Visa, MC, Discover, Amex) After Hamilton Virtual School's contribution, the student is responsible for the remaining semester balance of $176 with a $30 registration fee. Registration & semester tuition is charged upon registration. Tuition does not cover any retail music teaching materials such as books or instruments.

Semester Commitment

 

The homeschool partnership is split into two, 16-week semesters. Semesters for the 2023-2024 school year run from August 23rd-December 16th, and January 20th-May 17th. At the end of each semester, students have the opportunity to participate in our bi-annual recital, completely free to your family.

Students are committed to attend the full semester of lessons and all payments are non-refundable. Any student who drops out of the partnership mid-semester will be responsible for paying the remaining balance in full for any lessons taken.

Cancellations and Make-Up Lessons

 

As a courtesy to your instructor, please contact the office via phone, email, or text if your child needs to miss a lesson. When a student misses a lesson, that lesson can be made up using one of the four options available in the Academy make-up system.

1. Group weekend make-up classes.

2. Online Live Video Lesson at the student's normal lesson time.

3. Recorded video lesson.

4. Sublease your spot to a friend or family member.

When a teacher needs to cancel a lesson, every effort will be made to find a guest teacher. If a guest teacher is unavailable, students receive a Free Lesson Certificate for a private make-up lesson.

When the Academy closes due to weather, Live Video lessons will be offered at your normal lesson time. When the Academy closes due to power outages, a Free Lesson Certificate (private make-up with your teacher or another) that can be used at any time will be offered.

Arrival Information

The school is not responsible for providing before or after care for students. Students are not to be left in the building for excessive time periods before or after their lessons. Parents with children under the age of 8 are asked to remain in the school building during lessons.
 

Care of Students and Family Members

 
A parent/sibling waiting area is available. Please respect the studio atmosphere and ask young children to use their indoor feet and voices. Younger siblings cannot be accommodated in the teaching studio during lesson time. Children must wait for their parents to pick them up inside the Academy. Please do not ask your child to meet you elsewhere. The Academy assumes no responsibility for children who leave unaccompanied.
 

Injuries, Thefts, and Damages

 
 
Parents, legal guardians of minors, students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time. This includes hearing loss or hearing damage. The Academy of Music in Grand Rapids and Jenison Academy of Music are not responsible for any damage done to vehicles or items stolen from vehicles.
 

Payment Information

Authorization Agreement for Payment

(I/We) do hereby authorize Music Moves LLC, hereinafter named the COMPANY to initiate recurring (debit or credit) entries to (my/our) (Credit/Debit Card Account) as indicated and named below as the depository financial institutions, hereafter named FINANCIAL INSTITUTION. (I/We) acknowledge that the origination of ACH transactions to (my/our) account must comply with the provisions of U.S. law. Furthermore, if any such debit(s) should be returned NSF, (I/we) authorize the COMPANY to collect such debit(s) by electronic debit, and authorize all of the above as evidenced by my signature below.

This authorization is to remain in full force and effect until the COMPANY has received written notification from me (or either of us) of its termination in such time and in such manner as to afford the COMPANY and FINANCIAL INSTITUTION a reasonable opportunity to act on it.
